Children's Repeat Prescription Form

Use the form below to request a repeat prescription for a child or young person who is an existing patient. Our Nurse Prescriber, Helen Latch, processes repeat prescriptions on Mondays and Fridays. If you are an adult patient, please use our Adult Repeat Prescription Form.

Please Allow 7 Days' Notice
Repeat prescriptions are processed by Helen Latch (NMC: 96I5943E) on Mondays and Fridays. Please submit your request at least 7 working days before you need the prescription — urgent requests may incur additional charges and are not guaranteed.

Your repeat prescription request has been submitted. Helen will process it on the next Monday or Friday. You'll hear from us by email when it's ready.
